Come fill out a Texas Health needs Assessment survey

Lake Palestine United Methodist Church (LPUMC) is opening its doors on April 3rd from 9 AM to 5 PM to anyone who is willing to fill out a Health Needs Assessment Survey being conducted by the Texas Department of State Health Services.

The East Texas Community Health Needs Assessment will provide important information that will be used to improve public health, medical care, mental health and substance abuse services in East Texas.

Those having internet access may fill out the survey online at www.dshs.state.tx.us/ easttexas.

The State Health Service has conducted forums around East Texas in early March (if you attended one of these then your voice has been heard). Believing many people have not been able to attend one of the forums, and desiring that their valuable input not be missed, copies of the survey will be available at LPUMC on April 3rd.

The deadline for providing input via the survey forms is April 6th, so we do not have much more time to get the information in!

Anyone unable to come to the church may call for home-based arrangements (903-849-2379, leave a message).

LPUMC is on FM 315 South, approximately 8 miles south of Chandler (the "church on the lake" at the second bridge).
